1. Acknowledgment of Risks:

I understand and acknowledge that SSLTD is a sensory gym for children which involves the use of certain equipment and engaging in individual and group activities which may involve physical movement, climbing, swinging, jumping, and interaction with sensory materials. While Sensory Safari takes reasonable steps to ensure the premises are safe, accidents may still occur.(Required)

Acknowledgment of Risks (continued):

I understand that participation in the play activities of SSLTD naturally involves risks including, but not limited to, slips, trips, falls, collisions, sensory overstimulation, and minor injuries. I accept these inherent risks on behalf of myself and Minor under my supervision. (Required)

I acknowledge that SSDLT shall take reasonable steps to ensure the premises are reasonably safe but cannot eliminate all risks and is not liable for injuries resulting from risks that are inherent, obvious, or voluntarily accepted. (Required)

SSLTD, its owners, directors, employees, and contractors shall not be liable for any injury, loss, or damage suffered by myself or the Minor in my care arising from the use of the premises.(Required)

 agree that I am fully responsible for supervising Minor at all times. I accept responsibility for ensuring my child uses equipment safely and as instructed.(Required)

2. Binding Agreement on Behalf of Minor: I agree that this Agreement is made on behalf of the Minor participant and that all releases, waivers, and promises herein are binding on the Minor participant. I represent that I have full authority as Parent or Legal Guardian of the Minor to bind them to this Agreement.(Required)

3. Assumption of Risk: I voluntarily accept and assume all risks associated with Minor’s participation in SSLTD activities. The Minor's participation is purely voluntary, and I elect to have Minor participate despite these risks.(Required)

4. Release and Indemnification: I voluntarily release, forever discharge, and agree to defend, indemnify, and hold harmless SSLTD and all Released Parties from any and all claims, demands, costs, damages, and causes of action in connection with Minor's participation by the Released Parties.(Required)

5. Responsibility for Damage:

I accept responsibility for any damage caused to property, equipment, or facilities by myself or the child/children in my care due to misuse, unsafe behaviour, or failure to follow staff instructions.(Required)

6. Awareness of Safety Rules:

I agree to follow all posted rules, staff instructions, and safety guidance. Failure to do so may result in removal from the premises without refund.(Required)

7. Legal Costs:

Should SSLTD or anyone acting on its behalf incur attorney’s fees or costs to enforce this Agreement, I agree to indemnify and hold them harmless for all such fees and costs.(Required)

8. Insurance Certification:

I certify that I have adequate insurance to cover any injury or damage that Minor may cause or suffer while participating in SSLTD activities, or else I agree to bear the costs myself. I also assume any risks related to Minor’s medical or physical conditions.(Required)

9. Governing Law and Severability:

This Agreement shall be governed by the laws of the Republic of Trinidad and Tobago. If any portion of this Agreement is found to be invalid, illegal, or unenforceable, the remaining provisions shall continue in full legal force and effect. Any lawsuits arising from this Agreement must be filed exclusively in Trinidad and Tobago.(Required)

10. Health Representation:

I certify that, to the best of my knowledge, Minor does not have a health condition that would make participation inadvisable.(Required)

11. Photo & Media Release

I understand that declining promotional photography will not affect my child’s eligibility to participate in the Drop-Off Service. (Required)

I also understand that Sensory Safari cannot guarantee that my child will not appear incidentally in photographs or videos taken by other patrons or visitors while on the premises. Sensory Safari is not responsible for photographs or videos independently taken or used by third parties, except to the extent that Sensory Safari is required by law or its safeguarding obligations to take reasonable steps in relation to inappropriate or prohibited photography or recording.(Required)

12. Food and Allergies

I am responsible for fully disclosing all known food and other allergies. I understand that Sensory Safari will take reasonable precautions concerning disclosed allergies but cannot guarantee an entirely allergen-free environment. (Required)

13. Confidentiality Agreement:

I agree not to disclose or use any proprietary SSLTD information learned while at the facility. I also agree not to use any photographs of the facility for commercial purposes without SSLTD’s prior written consent.(Required)

14. Non-Licensed Daycare Acknowledgment:

By signing below, I acknowledge that SSLTD is not a licensed daycare facility.(Required)
